Student Information
What is required to start school?
Copies of your child's latest physical exam by a doctor or advanced nurse practitioner (not a chiropractor) and his record of immunizations are required by state law to start school. Please call your doctor and ask them to fax a copy to JGS at 603-532-4091.
If your child has a chronic condition or allergy, please let me know right away.
If your child is currently in school without the proper paperwork, they will not be allowed to attend field trips until that paperwork is sent in.
Please complete a Health Office Permission Form - this gives the nurses permission to treat your child when they come into the Health Office.
If needed, Convenient MD in Keene (603-352-3406) performs low-cost physicals and immunizations.
Immunizations
A copy of your child's immunization record must be on file in order to start school. Your Pediatrician's office can fax the latest record confidentially to JGS at (603) 532-4091.
If you are applying for a Religious Exemption, this form must be completed and notarized.
Physical Exam
Must be current within one year - Please call your pediatrician's office and ask them to fax the latest physical confidentially to JGS at (603) 532-4091.

Medication Policy

A Medication Administration Form with physician signature & parental consent needs to be on file at school before any medications can be given by the School Nurse or a delegated person. Please contact the nurse for a copy of the form, or download the form from the SNAP Health Portal.
Meds should be in the original container, and no more than 30 days worth of medication.
Parent must drop off medication. Children are not allowed to deliver medications.
