Allergies and Chronic Conditions

Food Allergy or Sensitivity? 


Please let us know as soon as possible if your child has a food allergy or food sensitivity. JGS strives to assist the parent in managing the risks of food allergies.  
The Health Office will alert their teacher and Food Services, but it is up to the parents to determine what your child will eat that day. 
If your child has a true food allergy that will require a special diet while here at school, JGS will need a doctor's order so that Food Services can provide alternatives. You can also use this form and ask your doctor to sign it.
If your child eats breakfast or lunch at school, sit down with your child and review the menus.  The menus are available from the school's website here.
If your child needs medication after accidental exposure to the food, an Allergy Emergency Care Plan will need to be filled out each year

Asthma?


Does your child need an inhaler at school? If so, please remember that JGS will need new doctor's orders, including an Asthma Action Plan, at the beginning of every school year. Please do not send in an inhaler with your child; please bring it in or send it in with another adult.  

Epi-pen? 


Does your child have an allergy that may require an Epi-Pen? If so, JGS will need new doctor's orders, including an Allergy Emergency Care Plan, at the beginning of every school year. Please do not send an Epi-Pen to school with your child; medications must be brought to the school by an adult.
