Prescription medications can be given to your child in the Health Office if:
Your child's healthcare provider fills out the Prescription Medication Authorization Form (located below)
Prescription medication is given to the School Nurse in its original package from the pharmacy
It must have your child's pharmacy label on it
Over the counter (OTC) medications can be given in the Health Office if:
The Over-the-Counter Form (located below) is filled out by the parent/guardian and
The medication is brought to the Health Office in a new , unexpired, and sealed package
Students are not permitted to carry any medications on their person unless the required paperwork has been given to the School Nurse (RRPS policy #1029).
Adrenal Crisis Action/Medical Management Plan
If your child has Adrenal Insufficiency or Addison's Disease, and requires emergency medication, you must have the medication available for your child to receive at school. Please print the Adrenal Crisis Action/Medical Management Plan and take to your Pediatrician to have it completed. Return the completed form to your child's school nurse. Note: All prescription medication brought to the health office must have a pharmacy label for your child.
Severe Allergy Action Plan
If your child has a known allergic reaction to anything that requires an EpiPen, you must have the medication available for your child to receive at school. Please print the Allergy Medication Authorization form and take to your Pediatrician to have it completed. Return the completed form to your child's school nurse. Note: All Epi-Pens brought to the health office must have a pharmacy label for your child. Benadryl brought must be unopened & in its original packaging.

If your child takes an over-the-counter medication that is needed at school, please print the over-the-counter (OTC) form and return the completed form to your child's school nurse. If your child has Seizures, they require emergency medication at school. You must have the medication available for your child to receive at school. Please print the Action Plan for Seizures form and take to your Pediatrician to have them complete. Return the completed form to your child's school nurse. If you choose to exempt your child from vaccinations, the State requires an exemption form be completed for each child. After completion it must be notarized and sent to Santa Fe for consideration. If you file an exemption, you MUST also bring the school nurse a copy of the notarized form.
