Please complete the following registration form to enroll your student for the upcoming school year. The registration process should take approximately 30-45 minutes to complete and needs to be completed in one sitting as there is no option to save and return.

Before you start the registration process, here are the things that you will need to provide:

- Two separate proofs of address in the parent's or legal guardian's name (i.e. electricity bill, cable bill, mortgage statement, etc.)
- A government issued picture ID of the parent or legal guardian
- Student's birth certificate (parent's name must be on the certificate or court papers must be provided)
- Student's Social Security card
- Student's last report card
- Student's immunization records
If you are unable to submit these records electronically, you will need to make an appointment to drop off hard copies with the administration at the school office. Please call (318) 487-4252 to schedule an appointment. You may also mail the documents to
