Inhaler to be labeled clearly with pharmacy label on inhaler, not on the box only

IMMUNIZATIONS

Parents please check your student’s immunization record with the document "requirements" to be sure minimum requirements are met.

  • REQUIREMENT FOR 7th & 8th GRADERS

TDAP – within the last 5 years

MENINGOCOCCAL

  • CURRENT RECORD OF IMMUNIZATION

A current immunization record must be on file. When your student receives an immunization, be sure to get an updated record and bring it to the nurse. If you have questions or need help in locating a clinic that gives these immunizations please call the school nurse 512-464-6614. If minimum immunization requirements are not complete, your student may be withdrawn from school. Please don't wait until the last minute to bring in records, this causes a bottleneck and delays your student from going to class.


HEALTH SCREENINGS

  • VISION

7th Graders will be screened as group in the fall

  • HEARING

7th Graders will be screened as group in the fall

  • SPINAL

Girls will be screened two times, once at age 10 and again at age 12

Boys will be screened once at age 13 or 14

  • TEXAS RISK ASSESSMENT FOR TYPE 2 DIABETES

7th Graders will be screened as group in the fall

  • Miscellaneous Screenings

Hearing, Vision, Spinal & the Texas Risk Assessment for Type 2 Diabetes Screenings will be done during this year for all students new to the district. Please make notice in writing if you do not want your student screened by the district. If your student does not get screened by the district, a physician’s record of these screenings must be on file with the school nurse.
