Rise Up Health Clinics provides no-cost health services for children and adolescents at St. Paul City School.

A parent or legal guardian MUST sign the consent form for patients under the age of 18 years.

Services are provided at low to no-cost to families whether or not a student has insurance. Health insurance will be billed to help cover the costs of your child’s visit whenever possible, but you will not receive a bill from the clinic. Please ask staff for more information. All students will be provided healthcare services whether or not they have insurance.

A parent or legal guardian must sign the appointment consent for students under age 18.

A parent or legal guardian must attend a sports physical or wellness exam appointment with the patient.

If the student needs a sports physical, please complete the sports physical form and bring it to your appointment. These forms will also be available in the school office.
