Athletics Paperwork & Forms

In order to participate in athletics at Ririe Junior-Senior High School, a student is required to have necessary medical paperwork in accordance with the rules of the Idaho High School Activities Association, the Ririe School District, and the law of the State of Idaho. The paperwork required for each student will vary based on grade level, if the student is new to the district, or if this is the first time the student is participating on an athletic team.

Please print the paperwork needed and bring it to the first day of tryouts or practice. No student without the completed paperwork will be allowed to practice or try-out until all paperwork has been signed and submitted.

NEW STUDENTS & NEW ATHLETES

If you are a new student in the district or if this is your first time competing on an athletic team ever or first time within a 12 month period, you will need to have the following: 

SENIORS

If you are a senior and have competed on an athletic team at Ririe Senior High School as a junior, you will need to have the following: 

JUNIORS

If you are a junior, you will need to have the following:

SOPHOMORES

If you are a sophomore and have competed on an athletic team at Ririe Senior High School as a freshman, you will need to have the following: 

FRESHMAN

If you are a freshman, you will need to have the following:

8TH GRADE

If you are in 8th grade and have competed on an athletic team at Ririe Junior High School while in 7th grade, you will need to have the following: 

7TH GRADE

If you are in 7th grade, you will need to have the following:

6TH GRADE

If you are in 6th grade, you will need to have the following:

*Please note that the only sports that students in 6th grade are allowed to participate in is cross country, wrestling, and track & field.
