Athletics Paperwork & Forms

In order to participate in athletics at Ririe Junior-Senior High School, a student is required to have necessary medical paperwork in accordance with the rules of the Idaho High School Activities Association. The paperwork required for each student will vary based on grade level, if the student is new to the district, or if this is the first time the student is participating on an athletic team.

To know which forms a student needs to have, please use the following guide:

NEW STUDENTS & NEW PLAYERS

If you are a new student in the district or if this is your first time competing on an athletic team, you will need to have an updated and signed Physical Form from a physician and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

SENIORS

If you are a senior, you will need to have an updated and signed Interim Questionnaire and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

JUNIORS

If you are a junior, you will need to have an updated and signed Physical Form from a physician and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

SOPHOMORES

If you are a sophomore, you will need to have an updated and signed Interim Questionnaire and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

FRESHMAN

If you are a freshman, you will need to have an updated and signed Physical Form from a physician and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

8TH GRADE

If you are in 8th grade, you will need to have an updated and signed Interim Questionnaire and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

7TH GRADE

If you are in 7th grade, you will need to have an updated and signed Physical Form from a physician and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

6TH GRADE

If you are in 6th grade, you will need to have an updated and signed Physical Form from a physician and also a signed copy of the Ririe Junior-Senior High School Athletic Contract. Please bring both of these forms with you to the first day of practices and tryouts.

*Please note that the only sport that kids in 6th grade are allowed to participate in is cross country and wrestling.

**Due to low turnout, 6th grade girls are allowed to tryout for the 7th grade girls basketball team.
