Google Sheet: blank daily diary
Use the view-only original to make a private copy. The blank grid has space for 31 daily entries. There are no diagnostic scores or automated medical conclusions.
Markdown: copyable migraine diary template
Copy the table into your own private file. Keep personal health records out of public comments.
Printable PDF: customize the columns and layout
Use Claru’s free generator when you want paper notes.
Use Yes when a headache happened, No when you know there was no headache, and Unknown when information is missing. A blank entry is not a recorded headache-free day. Note timing, symptoms, medication name and timing, and effects on everyday activities. Ask your clinician which details and period they want you to record.
Context can help you describe what happened. An observation near an attack does not establish that it caused the attack.
Write down the period covered, recorded headache days, explicitly recorded headache-free days, unknown days, medication-use days, and questions you want to ask.
