Transition Planning is the preparation for life after high school and coordination of services to maximize the future of an individual with a disability

The Rockland Transition Consortium consists of representatives from Rockland County school districts, public agencies, non-profit organizations, and local colleges who are committed to providing a smooth transition for individuals with disabilities from school to work and/or post-secondary programs, children's services to adult services, and home to the community at large
