2. Letter of Intent - Required for ALL QCVA Students

3. QCVA Student Contact (7-12) Required for 7-12 grade students ONLY

4. Consent for Medical Treatment & Medical Information Form

5. QCUSD Policy Acknowledgement Form (Available in ParentVue)

6. Enrollment Packet WITH driver's license and current proof of residency (gas, water or electric bill) 

7. *Only students residing outside our boundary - Open Enrollment Form (District boundary)

8. Email OR Drop Off Complete Forms

9. Course Selection (Secondary Grades 7-12 Only)- Please include a snapshot of current courses

QCVA families with students in Grades 9-12 are able to borrow a chromebook.  Please review the document and requirements on the left.  Walmart offer devices at a discounted rate and the QC library offers Chromebooks and hotspots to go.  Please check the Device Information document for more information.
