Asking the 5 Whys
We believe the most effective way to solve a problem is to solve it structurally, which is why we encourage our team to always ask the 5 Whys.
The 5 Whys is a technique used to discover the root-cause of a problem and not just its symptoms. When faced with a problem or defect, simply ask ‘Why?’ five times. You’ll find the answer is often far removed from the problem itself, yet will help you address the problem structurally.
Using the 5 Whys requires us to be honest with ourselves and the team, especially if one or more of the five whys leads back to us. In every situation, our default system setting should be to respectfully speak the truth. If we feel we cannot be open and honest towards each other, we should be asking ourselves "why not?"
