This (HCAP) form is completed by a physician for students with allergies to provide instructions on how to manage and treat your child's allergic reactions.
The self-carry form below also allows student possession and self-administration of allergy or anaphylaxis medication as long as the provider and the parent agree. The school nurse will assess the student's ability to manage the medication and work towards independent self-administration as needed.
*Students MAY self-carry and self-administer their own emergency medications such as Epinephrine pens, Antihistamines and Albuterol Inhalers, etc. with signed permission by their medical provider and parent. If you would like your student to be able to self-carry and self-administer, please submit the form below.
This form is required for a student to carry an epi-pen, inhaler, or rescue meds during school or school-sponsored activities.
