Gabapentin 600 mg PO once

Norflex 100 mg PO

Celebrex 400 mg PO (do not order if Sulfa allergy)

Acetaminophen 1 g PO Q8 hrs (Not ordered as "while awake")

Celebrex 200 mg PO BID

Gabapentin 300 mg PO QHS

LidoPatch daily: Lidocaine 3.6% -menthol 1.25% patch

Methocarbamol 750mg PO TID

Oxycodone 5-10 mg PO PRN

IV opioid if not tolerating PO or breakthrough pain

Zofran 4-8 mg IV PRN

Colace, miralax, senokot, Dulcolax

x10 Q 1hr while awake

D/C at 6 hrs postop

Regular diet POD 0

POD 0: 1x in hallway

POD 1 until d/c: QID in hallway

Up to chair for all meals

Cleared by PT

Pain controlled with PO meds

Tolerating adequate PO intake

Ambulating without difficulty

Ability to urinate

No s/s infection

Understanding of d/c instructions
