      • Confirm they brought booklet

      • Remind importance of documenting in booklet

  • PIVs

      • Place 2 PIVs

          • #1-on IV pump at 25 ml/hr

          • #2- Hep-locked

  • Clearfast ***DOCUMENT IN DIMENSIONS***

      • CONFIRM patient drank 3 hrs before scheduled surgery time

  • PONV screening ***DOCUMENT PONV SCORE IN DIMENSIONS***

      • Ordered by anesthesia if needed/high risk

      • Scopolamine patch (high risk only)

          • Hold for >65 years of age

          • Hold for h/o acute angle glaucoma

  • Patients on long-acting opioids

      • Take their home dose morning of surgery

  • Multimodal analgesia-ordered by surgeon

      • Acetaminophen 1 g PO once

      • Gabapentin 600 mg PO once

          • Hold for patients >65 years

      • Norflex 100 mg PO

      • Celebrex 400 mg PO (do not order if Sulfa allergy)

***If any of the above medications are not ordered, or not ordered correctly, contact surgeon ASAP for correction (contact anesthesia if surgeon not available)***

  • VTE/DVT prophylaxis

  • Normothermia in preop

      • Warm Blankets

  • Hand Off

      • Confirm with CRNA ALL medications given in pre-op for safety precautions and to ensure no double dosing occurs
