Please don't forget that each athlete must have a current pre-participation physical on file, Rank One Paperwork, and a signed Athletic Handbook acknowledgement form in order to participate or tryout. PLEASE NOTE THE FOLLOWING ABOUT 24-25 SCHOOL YEAR:
Prosper ISD Athletic Physicals for the 2024-25 School Year
● All Incoming Athletes need a NEW physical for the 24-25 school year
● You must fill out all online forms including a new medical history form found on
the rank one website
○ All RANK ONE forms must be completed

Mandatory Athletic Participation Forms: Parents, for the 2024-2025 school year, the required athletic participation paperwork for the UIL and PISD will be completed using electronic forms handled via our Rank One software. All incoming athletes in 7th-12th grades are required to have these forms filled out prior to practice and UIL competition. Please follow the link https://prosperisd.rankonesport.com/New/NewFormList.aspx and complete the online forms as soon as possible. You do not have to create a Rank One account to complete the forms but you will need your student’s ID number. There are instructions on how to complete the forms on the Rank One webpage the link takes you to. The physical and medical history forms will be the only paper forms to print either from the Rank One site or HERE. The physical form will still need to be submitted to the campus your child will be attending prior to any activity.
