Mandatory Athletic Participation Forms: Parents, for the 2023-2024 school year, the required athletic participation paperwork for the UIL and PISD will be completed using electronic forms handled via our Rank One software. All incoming athletes in 7th-12th grades are required to have these forms filled out prior to practice and UIL competition. Please follow the link https://prosperisd.rankonesport.com/New/NewFormList.aspx and complete the online forms as soon as possible. You do not have to create a Rank One account to complete the forms but you will need your student’s ID number. There are instructions on how to complete the forms on the Rank One webpage the link takes you to. The physical and medical history forms will be the only paper forms to print either from the Rank One site or HERE. The physical form will still need to be submitted to the campus your child will be attending prior to any activity.
