2. Fill out the form above. (Only yellow highlighted areas are required). 

3. Send the form back to the appropriate contact above to begin the set up process. Also include:

• Drivers license for each signer and personal info sheet for each signer (included in form above). 

• Drivers license and personal information sheet for each company owner/beneficial owner owning more than %25 of business.

• Drivers license and personal information sheet for the controlling person listed on the beneficial owner information in the form above. 

• Confirmation on which accounts you need opened (Rental Trust, Security Dep. Trust, Business Operating, Etc.)
