• You do not need to submit Experience Verification. Your transcripts will suffice in lieu of this requirement as well at the student teaching (clinical practicum) requirement.

• Reciprocity Information: If you are applying for licensure with Nevada and currently have a valid, non-conditional, non-provisional DOE Certification from another state, you may be eligible for reciprocal certification. Reciprocal certification, in most cases, will waive the testing requirements for Nevada and allow NDE staff to do the evaluation based on a license-for-license basis.

Requirements that you need to submit with your application:

• Verification of License(s) (Step 4)

• ASHA Certification Verification (Step 5)

•  Government-Issued Picture ID (Step 6)

If you forget to upload a doucment to your online application, you can send the document to license@doe.nv.gov. You will not be able to upload the document to your online application once the application is submitted.

You will need to complete a Background Check for your licensure application.

Please submit FD-258 fingerprinting cards after you have submitted your Nevada DOE Certification application and payment.  

Instructions will be sent to you after you have submitted the application.

You can have fingerprints rolled onto FD-258 fingerprint cards at a local law enforcement agency or fingerprinting agency, which may supply the FD-258 fingerprinting cards (please call ahead and ask). 

If they do NOT have the FD-258 Fingerprinting Cards on file, you can order them on Amazon.

Please ensure Fingerprint cards are signed by both you and the official taking the fingerprints. Cards without both signatures cannot be accepted.

You do NOT need to enter an ORI number on the fingerprint cards.

Upload a PDF copy of valid, non-conditional, and non-provisional DOE Certification(s) from the state(s) you are currently licensed to your online application.

Upload a PDF copy of valid ASHA Certification to your online application.

Submit a PDF copy of one of following:

• Valid Driver’s License

• Valid Passport

• Also required are any court documents showing any previous names that may be on your transcripts (i.e., marriage certificate, divorce decree, or name change decree)
