Instructions:
NOTE: Prior/current fingerprinting for any other purpose will not satisfy this requirement as other entities are not allowed to share criminal history background check results. You will need to complete this specific background check.

You must pre-register for fingerprinting services through IdentoGO and use this service code to ensure the Board receives your results: 1KGH98
IdentoGO accepts these identification documents.
After you are fingerprinted, you will receive a confirmation receipt, which you must upload to your application before submitting it. (You can upload a copy of your IdentoGo registration sheet to your online application to satisy this requirement)
If you do NOT have an IdentoGo location near you, please follow the instructions listed below:
Step 1: Fingerprint Registration
You will register your fingerprints through IdentoGo
Enter Service Code: 1KGH98
Select "Submit a fingerprint card by mail" option
Complete the registration questions
Pay the registration fee
Save the registration confirmation (it should be sent to you via email) - You will need to submit this with your fingerprint cards
Step 2: Fingerprinting
Contact your local fingerprinting agency or local police station to set up a fingerprinting appointment.
When you speak with a representative, ask them if they have two (2) blank FD-258 fingerprinting cards on file. If they do, please complete your fingerprints on the fingerprinting cards the agency supplies.
If they do not have two (2) blank FD-258 fingerprinting cards on file, you can order the fingerprinting cards through Amazon and bring them with you to your fingerprinting appointment.

Step 3: Fill out the Fingerprinting Cards
Once you have been fingerprinted, bring the fingerprinting cards home with you and fill out your fingerprinting cards EXACTLY as instructed below.
Signature of Person Fingerprinted: Your signature
Residence of Person Fingerprinted: Input your address
Employer and Address: Leave Blank
Reason Fingerprinted: LICENSURE: Speech Language Pathologist
ORI: Leave Blank
Fill out all boxes asking for your personal information
Don't forget to fill out your Date of Birth (DOB)
Step 4: Mail Fingerprint Cards
Mail the fingerprint cards and registration sheet to the mailing address listed below:

Instructions:
Provide an official notification of information (Self Query) from the National Practitioner Data Bank. Please refer to the NPDB website for additional information. When you receive the "Response to your Self Query," you will need to upload it to your online application.
Instructions:
Upload, where prompted, a current Curriculum Vitae listing all periods of employment or unemployment (i.e., child rearing, research, etc.) from graduation from school to present.
The list must be in chronological order, include the month and year, and indicate the state/territory in which the employment occurred.

Applicants for a license as a Speech-Language Pathologist must possess a master's degree or higher in speech-language pathology from an academic program accredited by an accrediting agency approved by the State Board of Examiners in Speech Language Pathologist & Audiologists.
Please note: the online checklist will not list official transcripts submission as a requirement, but it is a requirement for licensure submission.

Please submit 3 hours of approved training by the Department of Human Services (DHS) on the topic of child abuse recognition and reporting is required.
We recommend completing this COURSE.
After you have completed the required course, the approved provider will electronically submit your name, date of attendance, etc. to the PA Board.
When you create an account and log in, please select the "Access Your Course" option
IMPORTANT
Select "PA Licensing Board" to the "How did you hear about this course?" question
Select "Yes" to the "Are you an Educator Seeking Act 126 and/or Act 48 Credit?" question
If you do not have a PPID Number, please select N/A
Only the Timed version of the course issues Act 126 certificate. The non-timed course will only issue an Act 31 certificate. Please complete the Timed version

Contact the licensing authorities of the states, territories or countries where you hold or have ever held a license, certificate, permit, registration or other authorization to practice a health-related profession (whether active, inactive, expired or current) and request letters of good standing/verification of licensure in that state or jurisdiction.
The letter must include the following: license issue and expiration date, license status (current or expired) and disciplinary standing.
