PCMS Sports Participation Requirements:

To Participate in sports at PCMS, all forms must be completed before the season begins.

Required Forms:

 Physical -  Must be original NCHSAA-approved form signed by Doctor. Faxes will only be accepted directly from Dr. office only.

Online Athletic Registration Form 

Optional forms:

Transportation - Complete the online transportation form for each sport, only if your child will leave from away games with someone else. You do not need to fill one out if you are taking your own child. This needs to be completed at the beginning of each season.
