Required Documents

Personal lines: All policies require a signed application, a UM/UIM form, and a signed SurePay form. You need to get a new one if you change the bank account. 

Commercial lines: A signed application. A no-loss statement is not required for new business. 3-year loss runs are required on existing business. 

To download the app: Log in to InsurPAS, search for the policy number, click Print, and select the forms you need by entering the number of copies, such as '1' for one copy

Required Steps if you cannot get application: contact underwriting to cancel the policy within 60 days. After 60 days, request non-renewal and ask the underwriter to use a cancellation message like 'Signed Application Not Received' to prompt action from the insured
