COE Sabbatical Leave Application
Leave requests are due to the Human Resources, Academic Employment Manager by third Friday in January.
Faculty Member – Fall Term
Submits the completed Application and Contract for Sabbatical Leave form to their Department Chair
Department Chair – Fall Term
Reviews application form
Attaches a memo indicating how classes will be covered during faculty member’s leave, or any justification/support required owing to exceptional cases.
If approved, signs application form
Sends application with attachments to the Dean.
Dean’s office (Senior Fiscal Officer) – NO Later than January 10th
Checks for completeness and accuracy.
Verifies applicant’s eligibility, service, and salary data
Sign and complete the verification box, submit form to Dean
Dean’s Office (Dean) – NO later than January 17th
Is the final approval of the leave. (No one in the Provosts office or HR needs to approve)
Signs the application form.
Dean’s Office (Operations Specialist)
Notifies the applicant, Dept Chair, and OAA of the approval/denial by sending them a scanned copy of the approval form
Put the original form in their personnel file
By third Monday in January – submit application form with attachments to the Academic Employment Manager in Human Resources (HR). The University HR office just administers the details by entering the information into their system, adjusting FTE and confirming that the applicant is qualified. They communicate back to us on an exception basis. We will not hear back from them unless there is problem.
After Sabbatical:
Faculty member submits report to Department Chair. (no later than four weeks after the beginning of the first term of return)
Dean forwards report to Provost’s office.
