Part 1: What is the Disability Verification Form?

The disability verification form (DVF) is a form that requires personal information about yourself and you need your audiologist to fill out Part B, which clearly outlines that you are a student with a hearing loss. Why is this important? The DVF is your ticket to get the Bursary for Students with Disabilities (BSWD) money for your disability-related expenses, as it says that the government recognizes you as a person with hearing loss.

Part 2: What supporting documents do I need for the Disability Verification Form?

As soon as you get a letter of acceptance from college or university...

1) Complete the OSAP application for the upcoming school year (e.g. If you get accepted in April 2019, then you should complete the OSAP application for Fall-Winter 2019/2020 school year)

2) Call the Accessibility Office and make an appointment to discuss accommodations and BSWD.

3) Download the DVF form here

4) Fill out Part A (personal information)

5) Fill out Part B (audiologist’s information). You may have to go see them in person to fill this out for you, or you can email it to your audiologist and ask him/ her to send you a scanned copy after they have filled it out.

6) Bring the completed DVF on the day of the appointment with your counselor

On the day of your appointment, make sure you bring….

· Completed DVF form

· Your most recent audiogram

· Your Individual Education Plan (IEP) from your high school

· Your letter of acceptance (make sure you look for your student number in your letter)

· Social Insurance Number paperwork

· Your citizenship (in case you were not born in Canada)

NOTE: It’s okay to bring a parent with you. I brought my dad with me when I first started university in Fall 2012.

Part 3: Is the Bursary for Students with Disabilities part of the OSAP application?

The Bursary for Students with Disabilities (BSWD) is a SEPARATE form from the OSAP application. Once you have finished the OSAP application, THEN you can apply for BSWD application.

You are not responsible to look for the BSWD document on the website. Your counselor will take care of the BSWD that meets your learning needs. That’s why you need to complete DVF before the appointment.
