ALL PLAYERS MUST complete the Interest Form even if you played soccer with the program in the past.

Community Health Centers ("CHC")

Clinic hours: Monday through Friday, 8 am to 5 pm (Primary Care); Monday through Friday, 8 am to 7 pm (Pediatrics); Monday through Friday 8 am to 5 pm and Saturday 8 am to 5 pm (Immediate Care).

The price depends upon a person's insurance coverage.  If a family has no insurance, they may apply onsite for Temporary Medi-Cal; and the exam fee is sliding (based on family income.)

• Sign in to Home Campus. Click here  (this is required along with physical form to participate in practices for ALL SPORTS) 

• Complete your physical form, make a copy, and attach it to your home campus account.
