Instructional Setting Appeal Forms

The Texas Education Agency has recently notified school districts they may require virtual students who are failing and/or not actively participating in class to be moved from virtual instruction to face-to-face instruction. In compliance with the TEA’s guidance, Pasadena ISD has developed procedures for returning these virtual students to campus. At this time, some students are no longer eligible for virtual instruction due to their failing grade(s) and they must return to face-to-face instruction beginning January 6, 2021 in order to attain credit for their courses.

La agencia de educación de Tejas (TEA) recientemente nos notificó que podremos requerir que los estudiantes virtuales que están reprobando y/o que no estén activamente participando en clases sean removidos de clases virtuales a instrucción de cara a cara. En cumplimiento con la directiva de TEA, el distrito de Pasadena ha establecido el procedimiento para que estos estudiantes vuelvan a clases en persona. En este momento, su estudiante no es elegible para seguir con instrucción virtual debido a la/s materia/s reprobada/s y tendrá que regresar a instrucción de cara a cara empezando el día 6 de enero del 2021 para poder obtener créditos en sus cursos.

Please email all completed forms to your student’s counselor or call the Counseling Center at (713) 740-0330 if you have questions. All appeal forms must be submitted by no later than Friday, December 18, 2020.

Envíe por correo electrónico todos los formularios completados al consejero de su estudiante o llame al Centro de Consejería al (713) 740-0330 si tiene preguntas. Todas las apelaciones deben ser sometidas antes del viernes, 18 de diciembre del 2020.

Steps for Appeal Process

  1. Complete Parent/Guardian Appeal Form

  2. If appealing based on a medical condition as outlined by the CDC, complete the Medical Certification High Risk Exemption Form (to be completed by a Health Care Provider).

  3. Email form(s) to your student's counselor and/or request a meeting by Friday, December 18, 2020.

Parent/Guardian Appeal Forms

Appeals containing the required medical certification which verifies the student, or another individual within the same household, has a high-risk medical condition, as outlined by the CDC, will be granted. The required Medical Certification for COVID-19 High Risk Exemption can be found below. If you are appealing the move from virtual instruction for your student, both the Parent/Guardian Appeal Form and the Medical Certification High Risk Exemption form must be completed.

Se otorgarán apelaciones que contengan la certificación médica requerida que verifique que el estudiante, u otra persona dentro del mismo hogar, tiene una condición médica de alto riesgo, como se describe en los CDC. La certificación médica requerida para la exención de alto riesgo COVID-19 se puede encontrar a continuación. Si está apelando el cambio de la instrucción virtual para su estudiante, debe completar tanto el Formulario de apelación del padre / tutor como el formulario de Exención de alto riesgo de certificación médica.
