This will guide you on the process of renewing or updating your affiliation with Maxicare. Read the instructions carefully and proceed to fill up the Affiliation Renewal and Updating Form found on the link on Step 3 below.
Step 1: Make a copy and download the following documents required for the affiliation renewal (For Anesthesiologist and Visiting Doctors, please put Not Applicable on the Clinic Schedule Template if not applicable)
Step 2: Prepare the following documents required for affiliation renewal and/or updating:
Copy of Fellow or Diplomate Certificate
Copy of Fellow or Diplomate Certificate for Sub-specialization
Copy of PRC License
Copy of PHIC License
BIR Certificate of Registration (Form 2303)
Copy of Bank Statement
Curriculum Vitae or Resume
Duly Signed Physician Affiliation Agreement
Step 3: Click here to register
Update your contact information, professional details, or billing details. Renew your affiliation with various HMOs all at the same time.
Step 4: Wait for the confirmation coming from respective HMO Affiliation Officer.
