This will guide you on the process of applying for an affiliation with PAHMOC. Read the instructions carefully and proceed to fill up the Affiliation Form for New Applicants found on the link on Step 3 below.
Step 1: Make a copy and download the following documents required for the affiliation (For Anesthesiologist and Visiting Doctors, please put Not Applicable on the Clinic Schedule Template if not applicable)
Step 2: Prepare the following documents required for applying a new affiliation:
Copy of Fellow or Diplomate Certificate
Copy of Fellow or Diplomate Certificate for Sub-specialization
Copy of PRC License
Copy of PHIC License
BIR Certificate of Registration (Form 2303)
Copy of Bank Statement
Curriculum Vitae or Resume

Step 4: Wait for the confirmation coming from respective HMO Affiliation Officer.
