UPDATED FLORIDA "Mini Itineraries" updated 3/14/25
STUDENTS who are staying and riding back with their parent/guardian must have paperwork submitted to Mrs. Kowalczyk no later than Wednesday, March 12.
PARENTS/GUARDIANS who are staying in Florida and keeping their student with them will pick up their student's luggage at 7:30am on Monday, 3/24 from the hotel lobby.
You may also choose either check out with your student on Monday, 3/24 at....
1. 7:30 AM from the hotel lobby.
OR
2. 7:30 PM at the Hard Rock Cafe in Universal Studios.
Please include your check out choice on your form (if not already submitted) or email your choice Mrs. Kowalczyk by 3/12/25.
Please note that the above times/locations are subject to change (though we will do our best to avoid that) due to unforseen circumstances. It is your responsibility to stay informed using the website and Remind and make any necessary adjustments. Thank you!
Remember - parent/guardian designated on the submitted checkout form must be present at the time of check out or the student can not be released...aka I have to SEE YOU WITH YOUR STUDENT at the designated time before they can leave. This is for your student's safety and there will be No Exceptions.

UPDATE 2/7/25 - Trip Form Info
FORMS: Five forms were distrubuted to all students on 2/7/25.
The RED, BLUE, GREEN and YELLOW form are all due Friday, February 14.
If you need to update or change any information(insurance change, medication change, etc) on these forms after the February 14 due date, please complete a new form. Students can get additional forms from their director.
The WHITE form should remain at home. Info for the forms are as follows:
OPS Permission to Administer Medication Form - RED FORM - This is due Friday, February 14.
- Please note, this form is actually intended for OPS office use so there are a few things that are not accurate for our trip purposes.
1. Over The Counter (OTC) Medication
- Please do not submit individual bottles of the following OTC medications: (acetemenophin(Tylenol), Ibuprofein(Motrin) and antacid(tums)). As indicated on the green form, these medications will be available for students from the nurse on the trip.
- YES do submit any other OTC medication in its original bottle/box with student's name and dosage (as needed is fine). Those with multiple medications must all be turn all medications in together in a sealed ziplock baggie with the students name on it.
2. Medications/bottles will not be held until the end of the year as stated in the form but will instead be available for parent/guardian pickup in the main office after the band returns from the trip.
3. One form for all medications is fine. Please use the back of the form if additional space is needed.

This form a signed agreement acknowledging that parents and students have read and agree to the items on the OHS Bands Trip Guidelines & Rules Agreement form (white).

- Yes, we know some of this is also on the red form. Please complete and submit both forms anyway. :)
If you need additional space, please try to find a space on the same form if at all possible. You may also attach any additional pages necessary.
- Students will be able to get their medications (both scheduled and as needed) from their chaperone and/or the nurse. Students should not have medications on their person (exceptions: epi-pens or emergency inhalers) . When will I see my remaining balance?
As indicated in the original document (above) please plan for a total balance remaining update from Bennett Travel 30 days prior to the trip.
How much does the trip cost?
As indicated in the original document, the trip price depends on the amount of participants and the room size. We have between 111-121 people attending so please refer to the 111 column then your student's room size when looking for information regarding total cost then subtract the amount you have paid from this amount.

Fundraising money will be applied to student accounts in a timely manner. Please do not pay in full until fundraising money has been applied.
What is the chaperone application process?
Any parent/guardian interested in being a chaperone on this trip must email Mrs. Kowalczyk by Friday, 8/23. Sending an email does not guarantee a spot as a chaperone so please wait for further communication before finalizing any arrangements.
All director approved chaperones will be contacted with information regarding completion of a Volunteer Screening Form as well as general information regarding chaperone responsibilities and payment information.
Please note that while this is a very fun trip and a unique experience, all chaperones will be WORKING - hard. The kids’ safety and experience are top priority and that will not be compromised. If that does not sound like a good fit for you, please do not inquire about being a chaperone.
