On this page, you will find information about re-enrolling your student (this applies to students who are new to the district as well), through Q Parent Connect, at Oak Park High School for the 2021-22 school year. Additional information about the 2021-22 school year will be shared with you by the district and school site via ParentSquare.

OPHS Specific Registration Information

What is Registration?

Registration is the term used to describe all activities leading up to and associated with the start of school. At OPHS, it is held over a 4 day period in August when all completed and signed forms are turned in, school ID photos are taken, student class schedules are distributed, and other students and parent activities take place which are necessary to ensure a smooth start to the school year. All students, including newly enrolled students, students on permits, and all returning students, are expected to participate in Registration activities.



Parents and friends may register for you, but IDs or Textbooks will only be issued to students who register in person.

11th Grade - Tuesday, August 3, 8:30 am - 11:30 am

10th Grade - Wednesday, August 4, 8:30 am - 11:30 am

Log in to Q Parent Connect (Click here for Q Parent Connect Login Instructions) and click on Re-Enrollment/Annual Notices in the upper right corner. Please update each section with the most current information.

    • Each primary contact needs a unique phone number and email address. Shared phone numbers or emails impact your ability to receive ParentSquare information.

  • Student #1 (additional OPUSD students will be listed below) - 1 per student

    • Health Information (including optional Medication Authorization Form)

    • Medication Authorization Form - MUST PRINT AND HAVE DOCTOR COMPLETE & SIGN Part 2

      • You will drop this form off with your student's medication when we return in-person.

    • Acknowledgments & Permissions (including Parent Handbook, Student Technology Acceptable Use Agreement, Food Service, Military Opt-out and Cal Grant for 11th and 12th grade students only
