On this page, you will find information about virtually re-enrolling your student, through Q Parent Connect, at Medea Creek Middle School for the 2020-21 school year. Additional information about the 2020-21 school year will be shared with you by the district and school site via ParentSquare.

    • Each primary contact needs a unique phone number and email address. Shared phone numbers or emails impact your ability to receive ParentSquare information.

  • Student #1 (additional OPUSD students will be listed below) - 1 per student

    • Emergency Contacts

    • Health Information (including optional Medication Authorization Form)

      • Medication Authorization Form - MUST PRINT AND HAVE DOCTOR COMPLETE & SIGN Part 2

        • You will drop this form off with your student's medication when we return in-person.

  • Acknowledgments & Permissions (including Parent Handbook, Student Technology Acceptable Use Agreement, Food Service)

  • Upload Documents (ex. Court Orders, scanned Medication Authorization Form, etc.)

Medea Creek PFA Information

The Parent Faculty Association (PFA) is a volunteer group of parents and faculty who work together to assist the school administration by organizing events and providing financial funding for activities that best benefit the students and faculty of Medea Creek Middle School (MCMS). Money raised through PFA membership and our annual No Frills fundraiser goes directly to enriching the educational experience of all students.

Last year's donations provided support throughout the school for every department. In addition, we covered stipends to support student clubs and other activities including WEB (Where Everyone Belongs), Science Olympiad, Math Counts, Intramural Sports, Mock Trial, History Day, Art Club and others as well as the outdoor ping pong tables, and new music classroom furniture. Your monetary donation to the PFA (through membership dues and donations to the No Frills campaign) as well as your contribution of time are critical for a successful year. We welcome everyone’s involvement and encourage you to learn more about MCMS and the ways in which you can contribute to our students’ success.

The Oak Park Education Foundation (OPEF) raises money from parents, individuals, and local businesses, and makes direct contributions to the District annually to benefit children enrolled in all levels of Oak Park schools. All donations are tax-deductible and help maintain the high quality of education for our children!
