Appointment Request Reminders
*Please review reminders closely before requesting any appointments*
As Dr. Howard continues to recover from surgery, ALL appointments will be by telehealth until further notice.

Appointment requests are available up to 45 days in advance.
If your desired appointment date is beyond the 45-day window, please note when your date will be open & return to the calendar at that time.
You should receive a "Your Booking is confirmed" email immediately after submitting your request.
This email indicates that your request has been successfully submitted for review.
If your request is accepted & scheduled, you will receive a text message from 67579.
If you provided an email address with your booking request, you will also receive an email from Doxy.me.
If your most recent visit was prior to 2/15/2026, you must sign Consent & Acknowledgements (https://adobe.ly/42uy9xL)
and complete Patient History (https://adobe.ly/4912qbb)
Failure to complete these tasks before your appointment may result in cancellation of your visit.
If you need to change the date or time of your appointment request, please either:
Use the Reschedule link in the booking confirmation email (see below), or
Email hello@cfmny.net to cancel your pending request and submit a new request
Failure to cancel an unneeded request prevents other patients from booking the date/time for which you are no longer available.
