Students must fill out this request form for their teacher recommendation. Upon receipt, the IB Coordinator will send a recommendation questionnaire to the teacher. The student does not need to follow-up, however it is recommended that they give the teachers a heads up that the request is coming. STUDENTS MUST BE LOGGED INTO THEIR OLGCHS GOOGLE ACCOUNT TO ACCESS THE FORM

Parent support and awareness of the IB Program is integral to student success. All parents must submit this form acknowledging their student's interest in pursuing the IB Diploma Program.

- How have two past experiences in your life influenced your desire to participate in the IB Diploma Program? (300-400 words)
- The IB Program aims to develop learners who have characteristics known as the “IB Learner Profile” Choose which attribute is your STRONGEST and explain why. Next, choose which attribute you NEED TO IMPROVE upon the most and explain why. (200-250 words)
