Athletic Registration Forms for 2024-25 should be completed during Annual Check-In this summer in Infinite Campus.

All student-athletes are required to have insurance. If you do not have insurance, we have Student Accident Insurance available to purchase:

• English - Student Accident Insurance

• Spanish - Student Accident Insurance

• 2024-25 Transportation Consent (complete this form to allow your student to drive themselves or with a parent/guardian, instead of taking athletic busses)
