RE 91 : Kidney – Haemorrhage – Desseminated Intravascular Coagulation
CASE HISTORY
Pregnancy normal. In the 7th month she was admitted to another hospital with continuous lower abdominal pain. A diagnosis of concealed accidental haemorrhage was made and a lower segment caesarian section performed. A stillborn baby was delivered and approximately 3 pints of blood were found beneath the placenta. She was treated vigorously but remained anuric from that time. She was transferred here and underwent dialysis. However, she developed a bleeding tendency and died from a cerebral haemorrhage 10 days after her concealed accidental haemorrhage.
PATHOLOGY
Kidney 170mmx115mmx50mm. The specimen consists of a half of one kidney. The capsule has been stripped. There is a zone , about 4-5mm thick, of opaque, cream-coloured necrosis running right around the surface of the kidney and dipping down into the intermedullary cortex. This pale zone of necrosis is demarcated from the adjacent living renal cortex by a band of intense vascular congestion and haemorrhage.
