Guidelines & Forms

Attached are the forms for MHSA pre-participation physicals for 2026-27. 

Remember, if your student participated last year as a 7th, 9th, 10th or 11th grader they should have had a physical after May 1, 2025. And if that is the case, they do not need a physical this year from a provider. However, you need to fill out the interim-medical form. 

If your student was a 6th or 8th grader last year, they will need the pre-participationphysical form completed prior to participating. This includes a physical examination by a provider. The MHSA has also published a memo concerning this process and it is attached as well. 

It is highly recommended that students, below 7th grade this school year, get a physical, but it is not a requirement. 

Also attached is the parent_guardian_concussion form that must be completed prior to participating.
