6th Grade Students: The Tdap vaccine is required for all students who are 11 years old and entering 6th grade. If your child's 11th birthday is after the commencement of the 2018/2019 school year, proof of the vaccine must be received no later than 14 days after your child's 11th birthday. In addition, all 6th grade students are required to have 2 varicella (chicken pox) immunizations and 3 to 4 Polio vaccines with the final polio vaccine given after 4 years old. 7th Grade Students: Meningococcal Vaccine is required for all students entering 7th grade. Please see the immunization tab for more information.


**New York State guidelines regarding physicals have changed. Students entering 5th and 7th grades and all students new to the district will be required to have a physical on file in the health office. Acceptable health certificates may be dated anytime within 12 months prior to the start of the 2018-2019 school year. It is recommended that this examination be scheduled with your family health care provider who knows your child best. Please have your child's doctor complete the form below or contact the health office if you would like your child to have a physical at school with our school physician. **IF A COPY OF A CURRENT PHYSICAL OR THE DATE OF AN UPCOMING PHYSICAL APPOINTMENT IS NOT RECEIVED, YOUR CHILD WILL BE SCHEDULED FOR A PHYSICAL AT SCHOOL ON MARCH 6, 2019 WITH DR. All students in 5th and 7th grades are required to have a physical on file in the health office dated no earlier than one year prior to the start of the school year (9/1/2017). If your child has a physical scheduled, please notify the health office of the appointment date. If a copy of a physical or a physical date is not received a school physical will be given.
