*Documentation of a vision screening


The following Immunizations:

5 DTaP/DTP

4 Polio

3 Hepatitis B

3 or 4 Hib

2 MMR

2 Varicella or MD documentation of having had the chickenpox

Every child must have the above requirements BEFORE the first day of kindergarten. If the child is not up to date on their immunizations, then by state law they must be excluded from school until the requirements are met. This is a state law, not just a school district policy and we must abide by the law.

* The school must be provided with documentation of a "passed" vision stereopsis screening, or, if the student failed the vision screening or any student diagnosed with a neurodevelopmental delay, documentation will be provided regarding a comprehensive eye exam by a licensed optometrist or ophthalmologist that includes a diagnosis, treatment, prognosis, and any follow-up treatment if necessary.

NOTE TO PARENTS: PLEASE MAKE SURE THE PHYSICIAN OR THEIR STAFF PERFORMS THE PROPER TESTING ON YOUR CHILD. THE REQUIRED TESTS ARE AS FOLLOWS: OCULAR ALIGNMENT AND STEREOPSIS ASSESSMENT USING THE RANDOM DOT E CARDS; THE OTHER TEST IS THE MASSACHUSETTS VISUAL ACUITY TEST.
