ALLERGIES AND MEDICAL CONDITIONS

Scent Free School

Life Threatening Allergens

(Epi-pens required)

PEANUts

A reminder to parents that if your child has a severe allergy and uses an Epi-pen it is important for you to make an appointment with your public health nurse to complete an anaphylaxis form to be submitted to the school. These forms help to alert all staff members to which students have allergies and where to locate their epi-pen in case of allergic reaction. These forms need to be updated each year to ensure the Epi-pen is not expired, and that school staff know where to locate the Epi-pen.

Also, if your child has a medical condition that would require any medical intervention you must also visit the local public health nurse to complete an information sheet for the school. Any medical condition that requires school intervention should be documented in this way. If you have any questions or concerns please contact, Ms. Vanessa Chafe, 709-944-7731
