Instructions for Obtaining Employment Certificate:
1. Please complete and sign Part 1 of Application for Employment Certificate Form .
2. Please submit a photocopy of either Birth Certificate or Passport, State-Issued I.D., Driver’s License, Non-Driver’s License I.D.
3. Please submit an original letter of good health from your Doctor stating that minor is “fit for employment” (medical exam must be recent, within 1 year from date of application).
4. Please submit the completed application and the documentation to workingpermits@Nestmk12.net for processing or drop off in room 337.
You will receive the Employment Certificate Card within 1-2 weeks from
the day you submitted all the necessary documents.
