What is the Transitions Program?
The Transitions Program transforms the way schools support students and families who are experiencing a mental/medical health disruption. There is an internal referral process and caregiver approval for access to this program.
Students who are struggling to attend and/or succeed in school due to a variety of psychological, psychosocial, and/or medical difficulties are provided counseling and academic support as recommended. The ultimate goal of the Transition Program is to support students to get to a place where they can rejoin/re-engage in their classes while receiving the necessary support to do so.

Megan Bonomolo, Program Administrator
Megan is an Assistant Principal at Pollard and is the building administrator for the Transitions Program.
Kathryn Flinn, Transitions Counselor
I have been a School Counselor for 14 years. Prior to that I was an elementary school teacher. I went into the field of education because I love seeing kids learn and evolve. I left the classroom to be a counselor because I wanted to focus on working with students as they navigate their social emotional health. I have worked with all grade levels; however middle school is my favorite! I am excited for a new year as I meet new students and also see how former students have grown!
