DAILY SYMPTOM CHECK STONY BROOK ELEMENTARY SCHOOL STAFF AND STUDENTS 2021-2022

Monitor yourself and/or your students every day for symptoms. If you have any symptoms below, stay home, call your doctor and call your school nurse. **PLEASE CONTINUE TO MONITOR 4/2022**


Regardless of vaccination status, a combination of symptoms may

a COVID test prior to returning to school.

  • Fever (100.0° Fahrenheit or higher), chills or shaking chills

  • Difficulty breathing or shortness of breath

  • New loss of taste or smell

  • Muscle or body aches

  • Nausea, vomiting, or diarrhea

  • Cough (not due to other known causes such as chronic cough/allergies)

  • Sore throat

  • Headache (when in combination with other symptoms)

  • Fatigue, when in combination with other symptoms

  • Nasal congestion or runny nose (not due to other known causes, such as allergies) when in combination with other symptoms


If you or your student are staying home related to symptoms, have a COVID case in your home, are a close contact or receive a positive Covid test result. Please notify the nurse.
