Fill out the referral checklist and leave it in my box. I will put one in your box soon after receiving the email, or you can simply download it here.
I will contact you to schedule an observation (or two).
I will consult with you on intervention strategies and accommodations to trial in the classroom and send home parent permission to screen the student.
Once permission is obtained, I will contact you to schedule a screen with the student.
I will analyze and synthesize the results of the screen, and provide you and the parent with a write up, including my recommendations, which may be any of the following:
No occupational therapy services are recommended
I will recommend intervention strategies and accommodations to trial in the classroom and consult with you on their effectiveness, trying new strategies as needed, and/or moving to RTI or evaluation referral
I will recommend the student for Response to Intervention group
I will develop an RTI plan and send home permission to the parents
Once permission is obtained, I will consult with you on scheduling intervention times (this could be either pull out or inclusion).
After a set number of weeks, I will analyze and synthesize progress and provide you and the parents with a write up.
If issues prove persistent, I will continue RTI for another set number of weeks, with new strategies as needed and in a different setting as indicated, or move on to evaluation referral.
I will Recommend the student for occupational therapy evaluation.
I will fill out a referral form and Mrs. Ambrose will send it home to be signed.
Once permission is obtained, I will consult with you to schedule another observation, and times that I can pull the student for testing.
More than likely, I will send you a Sensory Processing Measure questionnairre to determine sensory processing issues.
If the student is found eligible, I will consult with you on goals and optimal intervention session times.
Once the IEP is signed, I will begin servicing the student.
