Complete the Club Sign-Up Form for each student by Monday, October 5. Clubs start on October 19. 

Review each club's information before completing the form; some clubs have additional acceptance criteria.

Please note that completing the sign-up form does not guarantee a student's acceptance to a club. If your students are accepted, the club advisors will contact you with club-specific information. 

Duplicates will not be accepted. The most recent entry will be considered your final preference.

Additional clubs are offered at CSPES this school year. The details for these clubs are not included in this document, as the club advisors have already communicated the information and selection process to the students and their families.
