In order to have medications at school, Parents must provide the following for ALL medications
Doctor/Provider order dated during the current school year
Pharmacy Labeled medication or Medication with labels for Over-the-counter medications
Medication Authorization form with Parent signature
Conditions that may require medications at school.....
