Nursing Assistant Youth Apprentice Opportunities 

Certified Nursing Assistant (C.N.A.) 

JOB SUMMARY: 

The CNA provides nursing care to residents, which includes all activities of daily living (ADL’s) such as personal  hygiene, eating, toileting, mobility and dressing. They also participate in providing restorative nursing care so  that the resident may achieve and maintain their highest level of functioning. 

ESSENTIAL JOB FUNCTIONS: 

1. Encourages and models team approach to accomplish the Luther Manor mission of providing  excellent care and services to customers (assists others as needed). 

2. Follows state and federal regulations, as outlined in the Employee Handbook, Nursing  Department Policy and Procedure Manuals, Human Resources Manual, and Resident Bill of  Rights, and adheres to safety rules. 

3. Reports potential/actual safety hazards to Nurse Manager/Nursing Supervisor and/or Security  immediately. 

4. Performs direct resident care per policy and procedure and resident plan of care: 

a. Resident baths (follows bath day checklist). 

b. Oral Hygiene. 

c. Ambulation. 

d. Range of Motion. 

e. Toileting. 

f. Bowel and bladder training programs. 

g. Resident grooming including: nails, hair, clean and appropriate clothing, shaving, etc. h. Answers call lights and calls for help and responds to alarms quickly. 

i. Restorative nursing per plan of care. 

5. Assists resident to have nutritional needs met per plan of care: 

a. Helps residents who require assistance with feeding. 

b. Supplies fresh water and other fluids as needed/scheduled. 

c. Offers HS snacks daily. 

d. Reports changes in appetite and intake to the nurse. 

e. Obtains accurate I & O’s and reports to the nurse, as needed. 

f. Obtains residents’ weights, as scheduled. 

6. Maintains/uses appropriate safety standards following resident plan of care: 

a. Follows resident’s plan of care for all transfers, including stand pivot transfer, EZ Stand  Transfer and EZ Lift transfers, performed according to the Policy and Procedure Manual.  b. Utilizes appropriate body mechanics continuously. 

 c. Uses gait belt for assisted transfers/ ambulation of residents. 

d. Positions residents per individualized schedule and plan of care to maintain body  alignment, decrease pressure and provide comfort. 

e. Conducts safety rounds at the beginning, during and at the end of the shift.

7. Practices infection control measures per policy and procedure: 

a. Provides skin care following the Skin Care and Wound Management Protocol. b. Disinfects and stores resident’s supplies/equipment and replace supplies as needed. c. Disinfects bathrooms and shower/tub rooms between resident cares. 

d. Handles linens appropriately, keeping clean linen separate from soiled linen. e. Follows Standard Precautions. 

f. Performs hand washing/personal hygiene. 

g. Follows Isolation precautions. 

 8. Sets priorities and organizes workload to complete resident’s care activities in a timely manner.  9. Establishes and maintains effective communication with staff, residents, families, visitors, and  volunteers. 

 10. Attends all mandatory in-services. 

11. Attends/participates in continuing education opportunities to meet 12-hour federal requirements.  (Must provide documented proof of outside education). 

12. Assists with orientation of new CNA staff. 

13. Reports resident care provided to team leader prior to completion of shift. 

14. Immediately reports resident changes of condition to team leader. 

15. Maintains resident confidentiality. 

16. Completes CNA documentation prior to completion of shift. 

17. Facilitates 1:1 or group activity with resident(s). 

18. Participates in QA activities periodically. 

19. Other duties as assigned.  

JOB REQUIREMENTS (Qualifications, Skills and Physical Demands) : 

1. Must be currently certified with State/Federal status on the State of Wisconsin Nursing Assistant  Registry. 

2. Must have the ability to interact diplomatically and effectively with staff, residents, family members,  visitors and volunteers. 

3. Must have the ability to communicate effectively and efficiently with residents and staff. 4. Must have the ability to read, write and communicate using the English language. 5. Must have the ability to spend 90% of the work shift moving about the work area. 6. Must have the physical strength to move or assist residents following the plan of care. 7. Must be able to push, pull or lift up to 50 pounds. 

8. Must follow Nursing Department Dress Code at all times. 

9. Must be able to pass routine or periodic drug testing per policy. 

10. Actively participates in or initiates problem solving for job-related issues.  

11. Works positively with coworkers, sharing job responsibilities when assignments are completed  and in times of shortage and special projects. 

JOB HAZARDS/ATMOSPHERIC CONDITIONS: 

1. Duties performed routinely require exposure to blood, body fluids and tissue. 2. Job tasks require intermittent exposure to wet and/or humid environment, especially during personal hygiene/bathing activities. 

3. Job tasks intermittently expose worker to chemical hazards, such as disinfectants, antiseptic  wipes, etc. 

4. Job duties intermittently expose worker to unpleasant odors/fumes. 

EQUIPMENT AND TOOLS: 

1. Routinely uses gait belt for safe resident transfers and ambulation; and uses EZ Stand or EZ Lift for  safe resident transfers, per POC. 

2. Regularly uses disinfectant for cleaning and hand washing. 

3. Routinely uses scale to weigh residents. 

4. Regularly moves residents in wheelchairs, and other modes of manual transportation. 5. Routinely assists residents with feeding utensils, per POC.

6. Regularly assists resident with use of commode, bedpan, urinal or other means of toileting. 7. Routinely positions resident in bed using linens, pillows, positioning aids, features of bed, etc. 8. Uses safety equipment per POC, including alarms, safety mattresses, transfer bars, call lights, etc. 9. May use specialty products including air mattresses, CPM machines, adaptive equipment and other  equipment, per POC.
