How to Entroll

A TOTAL OF FOUR (4) ITEMS MUST BE RECEIVED BY March 2, 2020

(The UH Online Application - please take a screenshot after submitting, The Dual Credit Application , The Health Clearance Form, and course payment)

Only use your FULL, LEGAL name (FIRST, MIDDLE & LAST) on all applications

Step One: Please complete the Online UH Application Form

Please click on the following link: https://apply.hawaii.edu

Select Leeward on the right side of the screen
Select "Running Start, Early Admissions, and Jump Start (non-degree)" as your major
Please take a screen shot and/or print the page after submission as proof of completion
You should receive a welcome e-mail from Leeward Community College

Step Two: Please complete the Dual Credit Application

Please click on the following link: www.hawaii.edu/dualcredit

Make sure it has you and your parent's signatures

I will get your counselor and principal signatures

Please submit to Dr. Miyahira

Step Three: Turn in your Health Clearance Form with proof of the following vaccinations:

Measles, Mumps and Rubella (MMR) - 2 doses
Tetanus-diphtheria; pertussis (Tdap) - 1 dose
Varicella - 2 doses
Tuberculosis Clearance

Step Four: Enrollment Fees will be made payable to Mililani High School. Tuition total is TBA. Tuition fee will be due on TBA (date will be updated as soon as Leeward publishes information. Please make checks payable to Mililani High School and turn checks in to Dr. Miyahira in H building offices

Tuition Breakdown:

Hawaii Resident Tuition - $131.00/credit hour
Student Activity Fee - $1.25/credit
Board of Student Communication Fee - $5
Student Health Center Fee - $15
