This waiver is a legally binding document required for all participants in the Miami Futbol Factory Youth Training Camp. It outlines the risks associated with physical activity, our medical disclosure policies, emergency procedures, refund terms, supervision guidelines, and media release rights.
Because your child’s safety is our top priority, we ask that all parents or legal guardians carefully read and understand the full waiver before signing during the registration process. This ensures full transparency around our responsibilities, your rights, and the expectations we have for participation.
The entire liability agreement is included within the official registration form and must be checked and electronically signed in order to complete enrollment. Signing this waiver constitutes a legal acknowledgment and full agreement with all terms and conditions stated.

FULL INJURY WAIVER & RELEASE OF LIABILITY
This Injury Waiver and Release of Liability Agreement ("Agreement") is entered into by the undersigned parent or legal guardian ("Guardian") on behalf of the minor participant ("Child") and applies to all activities associated with the Miami Futbol Factory Youth Training Camp ("Camp").
1. Assumption of Risk and Acknowledgment
Guardian acknowledges that participation in the Camp involves physical exertion and carries inherent risks of injury, illness, or in rare cases, death. These risks include, but are not limited to: sprains, fractures, heat exhaustion, asthma attacks, allergic reactions, and complications from pre-existing medical conditions.This Agreement expressly includes injuries or incidents arising from Acts of God, including extreme weather, natural disasters, or other unforeseeable events that occur on or during the duration of camp activities, whether on-premises or off-site.1. Guardian agrees to fully disclose any and all known medical conditions or concerns that may, in any way, affect the Child’s ability to safely participate in physical activity or be present in an outdoor environment. This includes, but is not limited to: respiratory conditions (e.g., asthma), severe allergies, cardiovascular concerns, neurological conditions, photosensitivity, heat sensitivity, or prior injuries.This requirement is not limited to the examples provided and extends to any condition—physical, psychological, or environmental—that may impact the participant’s health, safety, or ability to fully engage in the program.If the Child requires medication, medical devices (e.g., inhalers, EpiPens), or emergency care protocols, it is the Guardian’s responsibility to inform Miami Futbol Factory staff and coaches in writing prior to participation.Failure to disclose such conditions may result in removal from the program with no refund issued. Miami Futbol Factory reserves the right to make unilateral decisions regarding participation based on health risk assessments.2. Medical Disclosure Requirement
3. Medical Treatment and Financial Responsibility
Should the Child require medical attention—including but not limited to CPR, first aid, ambulance transport, hospitalization, or any form of emergency treatment—Guardian agrees to accept full financial responsibility for all associated costs and to indemnify and reimburse Miami Futbol Factory for any expenses incurred on behalf of the participant.The Camp does not provide medical insurance. Guardian hereby waives, releases, and holds harmless Miami Futbol Factory, its staff, owners, representatives, volunteers, affiliates, and associated personnel from any and all claims, demands, causes of action, or liability for personal injury, illness, disability, or death arising directly or indirectly from participation in the Camp.This release includes but is not limited to negligence, equipment failure, facility conditions, weather, or unforeseeable accidents.4. "Acts of God" refers to any external event or force that makes it impossible for a player to participate in a scheduled training session and is entirely beyond the control of the participant or Miami Futbol Factory. This includes, but is not limited to: 5. Acts of God and Credit Policy
- Severe weather or natural disasters
- Field closures mandated by the city or facility authorities
- Medical emergencies that result in hospitalization or leave the player physically incapable of any physical activity (e.g., bedridden, unable to walk, run, or move independently)
- Proof must be submitted directly to the coaches, either in person or via email.
- If approved, a credit will be issued and can be used for any current or upcoming eligible training session.
- If no suitable session is currently available, the credit will be held in our system and the family will be notified when eligible future sessions become available for use.
6. Refund and Cancellation Policy
All registrations are non-refundable. If a disclosed or undisclosed medical condition results in the Child being unable to continue participation, no refund will be given. Exceptions are only made in the form of future credit if the issue arises from a documented, camp-related injury due to an Act of God.6. Refund and Cancellation Policy

This Agreement is binding upon signature and shall remain in effect indefinitely, covering all future participation in any Miami Futbol Factory programs unless revoked in writing. Guardian agrees to forfeit all rights to sue or bring legal action against the Camp, its staff, owners, or affiliates.7. Duration and Legal Binding
8. Drop-Off, Pick-Up, and Session Attendance Policy
Miami Futbol Factory is not responsible for supervising children outside of the designated time window surrounding each training session. Specifically:8. Drop-Off, Pick-Up, and Session Attendance Policy
- Drop-off supervision begins no earlier than 15 minutes before the scheduled start of the session.
- Pick-up supervision ends 15 minutes after the session concludes.
- If notice is properly given, the participant will be eligible to receive a credit to attend a comparable make-up session on a future date, subject to availability.
- Failure to notify at least 24 hours in advance will result in the forfeiture of that session without credit or refund.
9. Dress Code & Camp Kit Policy
All campers are required to attend each training session in full Miami Futbol Factory-approved training attire. This dress code promotes safety, discipline, and unity during all activities.Campers registered under the Full Retail Camp package will receive a complete camp kit, which includes: 1 training jersey.Participants must arrive at every session in the appropriate uniform. Failure to adhere to the dress code may result in the camper being withheld from participation for that session until compliance is met. Repeated violations may result in removal from the program without refund.Additional kits or individual uniform items (jersey, shorts, socks, or ball) may be purchased separately upon request. 10. Media Release and Rights Assignment
By signing this waiver, the undersigned parent or legal guardian hereby grants Miami Futbol Factory, its agents, representatives, affiliates, successors, and assigns, the irrevocable and perpetual right and license to use, reproduce, display, distribute, publish, edit, and otherwise market, in any form now known or hereafter developed, any photographic, video, audio, or other media recordings of the participant captured during the Miami Futbol Factory Summer Training Camp by staff memebers.Such media may be used for, but is not limited to, promotional, advertising, social media, commercial, and educational purposes, without further notice, approval, or compensation to the participant or their legal guardian.The undersigned further waives any right to inspect or approve the final use of said media and understands that all such media is the exclusive property of Miami Futbol Factory. This includes a full waiver of any and all claims to copyright, trademark, publicity rights, or other proprietary rights related to the media content.If a parent or guardian wishes for media containing their child to be excluded from public use, they may submit a written request via email or speak directly with camp staff. Upon receipt of such a request, Miami Futbol Factory will make reasonable efforts to remove, obscure, or refrain from publishing identifiable media involving the participant in future materials.The undersigned agrees that no financial compensation, residuals, or share of revenue derived from the use of such media shall be owed to them or the participant, and no legal claims may be made against Miami Futbol Factory for the use of said content, including claims of copyright infringement, misappropriation, or violation of privacy.10.
