Esports Volunteer Information

In order to volunteer you will need to do the following:

Complete a background check.

1. A background authorization form (PERS 944) needs to be filled out and returned to Marissa Held. This form can be turned in to the Main Office at Thomas Metcalf or emailed to mvheld@ilstu.edu.

2. An instruction sheet with information on where to go to get your fingerprints taken.

   • Fingerprinting Instructions

3. A Biometric Impressions Authorization Form. This only needs to be completed if you are going to Biometric Impressions for your fingerprints.

4. Complete a Volunteer Risk Assumption Form - signed and turned in to Marissa Held.

5. ISU Volunteer and Reporting Form - signed and turned in to Marissa Held.

You may start your volunteer work only after the requirements above are completed AND you are notified by Marissa Held from the Metcalf main office.
