Certifies that the patient has a terminal illness and cn remain the patient's prmary physician and work as a member of the hospice team.

Certifies the attending physician's prognosis, leads the hospice team in developing a plan of care, provides consultation to the attending physician.

Evaluates the patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques, the use of medication, and pain management.

Asseses social needs of the patient and family, and provides psychological and bereavement counseling.

Provides spiritual support, works as a liaison with clergy of the patients choice.
